JOB TITLE: Transition Specialist
WAGE/HOUR STATUS: Exempt
PAY GRADE:A03
REPORTS TO: Director of Special Education
DUTY DAYS: 198 days
DEPARTMENT: Special Education
DATE REVISED: June 2020
This position is eligible for a Chapter 21 contract.
PRIMARY PURPOSE:
Facilitate the overall implementation of transition planning and
services, assuring that
coordinated outcomes are appropriately planned for all students
with special needs, Provide
parents and teachers with appropriate tools and information
necessary to effectively plan and
deliver transition services.
QUALIFICATIONS:
Education/Certification:
Master's degree
Valid Texas teaching certificate in Special Education
Valid Texas educational diagnostician certificate preferred
Completion of/ability to complete required professional development
hours in vocational training with students with special needs as
required by law
Special Knowledge/Skills:
Excellent communication, organizational, and interpersonal
skills
Ability to interpret data
Knowledge of current regulations governing transition services for
students with disabilities
Ability to work with various community and governmental
agencies
Experience:
Minimum three years teaching experience in Secondary Special
Education (preferred)
Minimum Experience:

Safety: Contribute to the prevention of accidents and injuries by
observing safety rules and District policy, practicing the
principles and skills taught in safety training, wearing personal
protective equipment as required, reporting injuries and incidences
immediately to supervisor, and being proactive in the ongoing
efforts to improve and maintain workplace safety.
