JOB TITLE: Transition Specialist

WAGE/HOUR STATUS: Exempt

PAY GRADE:A03

REPORTS TO: Director of Special Education

DUTY DAYS: 198 days

DEPARTMENT: Special Education

DATE REVISED: June 2020

This position is eligible for a Chapter 21 contract.

PRIMARY PURPOSE:

Facilitate the overall implementation of transition planning and services, assuring that

coordinated outcomes are appropriately planned for all students with special needs, Provide

parents and teachers with appropriate tools and information necessary to effectively plan and

deliver transition services.

QUALIFICATIONS:

Education/Certification:

Master's degree

Valid Texas teaching certificate in Special Education

Valid Texas educational diagnostician certificate preferred

Completion of/ability to complete required professional development hours in vocational training with students with special needs as required by law

Special Knowledge/Skills:

Excellent communication, organizational, and interpersonal skills

Ability to interpret data

Knowledge of current regulations governing transition services for students with disabilities

Ability to work with various community and governmental agencies

Experience:

Minimum three years teaching experience in Secondary Special Education (preferred)

Minimum Experience:

• Excellent organizational, communication, and interpersonal skills
• Ability to operate computer/peripherals using a variety of basic applications
• Ability to effectively use district programs with reasonable assistance
  
  MAJOR PERFORMANCE RESPONSIBILITIES AND DUTIES:
  
  
• Meet with age appropriate students and their parents each year to develop or review outcomes in the areas of education, community involvement, employment, and daily living
• Coordinate transitional activities with state agencies for students as appropriate
• Serve as a resource for training students with disabilities at the high school level on disability awareness, self-determination, and self-advocacy
• Plan and conduct student/parent training sessions on accessing transition services
• Identify IEP goals which will lead to a coordinated set of activities for post-secondary outcomes
• Evaluate students regarding their functional vocational skills
• Provide training and technical assistance to teachers and other staff to assist them in planning for and delivering effective transition services
• Coordinate the integration of the academic curriculum into transition services
• Provide assistance to teachers and other staff members in order to facilitate the provision of effective transition services
• Assist Special Education Department with evaluations as needed between school sessions if Educational Diagnostician certification held Program Management:
• Develop and maintain effective relationships with students and parents
• Compile, maintain, and file all physical and computerized reports, records, and other documents required, including student records, test results, and test inventories
• Comply with policies established by federal and state law, State Board of Education rule, and local board policy in the areas transition services, assessment, placement, and planning for special education services
• Comply with all district and local campus routines and regulations Communication:
• Maintain a positive and effective relationship with supervisors
• Effectively communicate with colleagues, other school personnel, students, and parents Professional Growth and Development:
• Take the initiative and participate in professional development activities to improve skills related to job assignment
• Act in a professional ethical manner as outlined in the Standards of Conduct for the State Board of Educators and the Standards of Conduct for the Teaching Profession School/Community Relations:
• Communicate and convey information with clarity regarding the district's programs
• Act as an effective liaison among staff, parents, and community
• Involve parents and the community in district and special education program improvement efforts as appropriate
