Responsible for assessment, initiating individualized care plan and developing medication reconciliation.What will you do in this role
    • Performs behavioral, chemical dependency, current and historical medical evaluation Admission Assessment
    • Develops and complies individualized Plan of Care based on patient-centered model and standardized criteria sets.
    • Collaborates with physicians, nursing staff, internal medicine and dietician.
    • Provides resource information for all staff.
    • Referrals for specific medical conditions requiring dietary consultation What qualifications you will need:
      • Basic Cardiac Life Support must be obtained within 30 days of employment start date
