LifeSpan Home Care , a trusted name in home health services, is seeking a PAS Case Manager to join our dedicated team in Fort Worth, TX . About the Role: As a Case Manager , you’ll play a vital role in coordinating and supervising personal assistance services across a variety of programs, including: · STARPLUS · Primary Home Care · Family Care · Community-Based Alternatives · MDCP · Private Duty/Personal Assistance/Sitter Services You’ll ensure high-quality care is delivered in accordance with physician plans, state program guidelines, and agency policies—while also providing hands-on support when needed. Key Responsibilities: · Supervise PAS/community care staff and coordinate patient care · Manage billing, scheduling, and office operations · Conduct timely supervisory visits and maintain accurate documentation · Collaborate with HR, payroll, and external caseworkers · Ensure compliance with regulatory and agency standards · Support hiring, training, and performance tracking of care staff · Participate in budget planning, meetings, and quality improvement initiatives ?
